Patient Portal

The Nantucket Cottage Hospital Patient Portal is an online health management tool that allows patients to view a summary of their hospital visit. This record includes a view of clinical data from patients’ electronic medical record during their stay at NCH.

To register now for access to the NCH Patient Portal, please download and print the consent form and return it by checking-in at the NCH outpatient registration area at the hospital.

Some of the information patients will find in the portal includes:

  • Allergies
  • Immunizations
  • Active and inactive medication(s)
  • Procedures completed
  • Existing or developing medical conditions
  • Certain test results (i.e. lab results)

In addition to viewing this information online, patients will be able to download the information to their own private computer for their personal record. Patients will also be able to send this information to another healthcare provider as long as that provider has supplied them with their secure email address.

Patients can sign up with the NCH outpatient registration staff during regular business hours. Prior to registration, the patient should download, print and complete the consent form. If you are a parent or patient representative for someone under 18 years of age, please complete this form instead. Once registered, the patient will receive an email with a link to the Patient Portal site so they can set up an account.

Please note that some hospital visits may generate only limited information on the patient portal (i.e. Emergency Department). However, to gain access to the complete medical record, please contact the Medical Records Department at (508) 825-8282.
